What do I need to bring to the appointment?

Please bring along your referral from your doctor and your Medicare card, as well as your private health fund details.  You should bring along your medications or a list of your medications,  as well as X-rays or other imaging studies that you may have.  If you have diabetes, it is important that you bring along your glucometer and documentation of your home glucose monitoring  if you are already doing this at home.

Please arrive ten minutes before your appointment so that we can take down your details including address, referring doctor, and contact details.
