Your Appointment

On the day of your appointment:

Please bring along your referral from your doctor and your Medicare card, as well as your private health fund details.  You should bring along your medications or a list of your medications,  as well as X-rays or other imaging studies that you may have.  If you have diabetes, it is important that you bring along your glucometer and documentation of your home glucose monitoring  if you are already doing this at home.

Please arrive ten minutes before your appointment so that we can take down your details including address, referring doctor, and contact details.

What are the payment options?

Payment is expected at the time of the appointment.  We accept  Visa, Mastercard, EFTPOS, cash and cheque.

How to book an appointment:

Please ring for an appointment. If you have a condition requiring urgent review, please ask your doctor to contact us to determine when an urgent appointment is needed.

To cancel an appointment:

We request that you notify the practice 48 hours before your appointment, so that the appointment can be given to someone else.  Cancellation fees apply if less than 48 hours notice is provided.

For patients with a disability:

We have fully wheelchair accessible consulting rooms and bathroom facilities.
There are 2 disability carparks underneath the building and lift access to the consulting rooms.


There is usually ample parking in the car park at the back of the building in Date Street (for GPS or Google Maps, it is next door to 53 Date Street). There is also parking in the surrounding streets. For patients with a disability, there is disabled parking beneath the building and which is accessed via proceeding to the traffic light within the car park, waiting for the light to turn green then proceeding up the ramp.
